Family day care educator resignation form

Fill out this form to resign from a Department for Education family day care (FDC) scheme.

Business administration

If children are absent on the day(s) before you finish in FDC, the family will not be eligible for CCS and will be required to pay you the full fee.

You will need to cancel your subscription to Harmony Web for Educators after you have finalised all invoices and receipts.

Returning to family day care

If you choose to return to FDC you must contact the Business and Customer Support Centre (BCSC) to start the application process.

If you were working towards completing a workplan or if you are subject to a current investigation on the day of your resignation, this information will be considered in any new application to become registered again with a FDC scheme.

  • My registration as a FDC educator will be cancelled from the date indicated on this form.
  • I have contacted all families with at least two weeks’ notice to let them know that I am closing my FDC business.
  • As required by regulation 179 of the Education and Care Services National Regulations I will return the documentation of each child that I have educated and cared for as part of my family day care service to the department, including:
    • child assessments or evaluations for educational program delivery
    • incident, injury, trauma and illness records
    • medication records
    • the record of visitor’s to the FDC residence

  • I understand that if I wish to restart my FDC business that I will need to apply for a new registration.
  • I am aware that if I was working towards completing a workplan or I was subject to an investigation on the day of my resignation, that this information will form part of any new registration application.
  • I will remove any reference to my registration with the department, including advertising and registration conditions and certificate of my FDC business, from my premises, the community and online locations.
  • I am responsible for cancelling my Harmony web subscription.
  • I (as a small business operator) will retain all required records in accordance with the Australian Taxation Office.
  • I understand that if I provide private care, I may be in breach of the Education and Early Childhood Services (Registration and Standards) Act 2011.
