The concerns checklist outlines some prompts of actions that could be taken when we are responding to concerns about children and young people.
Provides guidance about the information that you should have ready when you make a report to the Child Abuse Report Line via phone (13 14 78) or online (eCARL).
Staff and volunteers must notify the child abuse report line if they suspect on reasonable grounds that a child has been or is being abused or neglected, according to the Children and Young People (Safety) Act 2017.
It is the responsibility of the site leader to keep records of mandatory notifications.
The Department for Education has a mandatory notification record template (DOC, 159.5 KB) (staff login required). Our staff are required use this form to record their notification, whether done by phone or online.
Staff must log on to the intranet to find out about notification and record keeping responsibilities.
This form has been developed for use by department sites. Other sites and services can include the elements of this record-keeping form for their own work.
